SanseraPopHealth™ · Population Health Analytics

Your whole panel, turned into action.

You're accountable for populations — quality measures, risk contracts, UDS tables, shared savings — but your data lives in six systems and your "analytics" is a spreadsheet someone updates quarterly. SanseraPopHealth™ turns your EHR, claims, pharmacy, lab, and SDoH data into one live picture of your panel: who's at risk, what's owed, what it costs, and what to do next — without hiring a data-science department.

The Problem

Enterprise pop-health tools weren't built for you

The analytics platforms serving billion-dollar health systems assume an enterprise data warehouse, a team of analysts, and a seven-figure budget. So community health centers, independent clinics, and mid-size hospitals run population health the hard way: quality gaps discovered when the payer report lands, risk scores nobody recalculates, UDS season as an annual fire drill, and VBC contracts signed on faith because nobody can see total cost of care until the reconciliation arrives.

The cost of flying blind is concrete: quality dollars forfeited, risk revenue under-documented, shared savings you earned but can't prove, and high-risk patients you find out about in the ED census instead of your worklist.

SanseraPopHealth was built for the organizations the enterprise vendors skip: everything computed for you, continuously, from data you already have — priced for safety-net economics.

What It Does

Four pillars, one live picture

Analytics — cost, quality, utilization

One dashboard that answers the questions your board, your CFO, and your payers actually ask. Total cost of care trended by payer contract, provider, site, and patient cohort. Utilization patterns — ED visits, admissions, readmissions, imaging — with the outliers surfaced, not buried in averages. Network leakage identified: where your patients go when they don't come to you, and what it costs. PMPM trends against contract benchmarks. Every view drills from organization to site to provider to the individual patient chart, because an insight you can't act on is a chart, not a tool.

What changes: leadership meetings run on this month's numbers instead of last quarter's exports, and "how are we doing on the Medicaid contract?" gets answered in one click instead of one week.

Risk stratification — HCC, CDPS, Charlson

Risk is the currency of value-based care, and most organizations are under-banked. PopHealth continuously scores every patient on the models your contracts actually use — HCC for Medicare risk adjustment, CDPS for Medicaid, Charlson for clinical comorbidity burden — plus utilization- and SDoH-informed composite scores that flag the rising-risk patient before the claims do. The AI layer does what a scoring engine alone can't: it reads encounter notes and claims history to surface documented-but-uncoded conditions — the diabetes with complications recorded in a note but coded plain, the CKD stage that never made the problem list — and queues each one for provider confirmation. Recapture alerts flag chronic conditions coded last year but not yet this year, before the documentation window closes.

What changes: risk scores that reflect the patients you actually treat, risk-adjusted revenue that reflects the work you actually do, and a rising-risk worklist that feeds SanseraCCM360™ enrollment automatically — the highest-value patients get managed, not just counted.

Quality tracking — HEDIS, MIPS, UDS, live

Quality reporting shouldn't be archaeology. PopHealth computes your measures continuously — HEDIS, MIPS, Medicare Stars inputs, and the full UDS clinical tables — from live EHR and claims feeds, with drill-down from the organization number to the provider scorecard to the exact patients in the denominator. Every open gap is a workable list, and every workable list can flow directly into a SanseraEngage™ outreach campaign — the gap report and the gap closure on the same platform. For FQHCs: UDS season stops being a season. Clinical tables compile continuously with validation and error-flagging, so HRSA submission becomes a review, not a reconstruction — and your quality team gets January back.

What changes: you see measure performance the week it moves, not the quarter after; provider scorecards drive improvement conversations with data nobody disputes; and 200+ staff hours a year come back from manual chart abstraction.

VBC performance — TCOC and shared savings

Value-based contracts are bets, and PopHealth is how you stop betting blind. Shared savings and loss projections tracked live against every contract's benchmarks. Total cost of care decomposed — where the dollars go, which cohorts drive trend, which interventions bend it. Scenario modeling that answers "what happens to our MSSP position if we cut readmissions 15%?" before you invest, not after. And attribution tracking so you know exactly which patients you're accountable for — the detail that decides reconciliations.

What changes: you walk into payer negotiations with your own numbers, take on risk you can actually see, and prove the savings you earned instead of hoping the payer's math agrees.

How It's Built

Answers, not homework

  • Runs on the Sansera data platform. EHR, claims, pharmacy, lab, ADT, and SDoH feeds unified once — PopHealth computes on the same complete patient picture every Sansera agent uses. FHIR R4 native, Bulk FHIR for panel-scale queries.
  • Measures maintained for you. Measure logic, risk-model updates, and reporting-year changes ship as platform updates — not as projects for the analyst you don't have.
  • Role-ready views. Executive dashboards for the CEO/CFO, provider scorecards for medical directors, prioritized worklists for care managers, deep-dives for quality directors, and board-ready exports that don't need a designer.
  • Insight with hands. This is the difference: on the same platform, a gap list becomes an Engage campaign, a rising-risk list becomes CCM enrollment, an attribution list becomes an AWV drive. Analytics that end in action, not in a PDF.
The Numbers

Live, not quarterly

Live HEDIS / MIPS / UDS computation — no quarterly lag
3 risk models maintained continuously (HCC · CDPS · Charlson) plus composite rising-risk scoring
200+ staff hours returned annually from manual abstraction and UDS compilation
One platform from insight → outreach → enrollment → billing
FAQ

SanseraPopHealth™ questions

How is this different from our EHR's population health module?

EHR modules see EHR data. PopHealth computes on EHR plus claims, pharmacy, lab, ADT, and SDoH — which is why its cost, utilization, and attribution views exist at all. And it's connected to agents that act on the lists, which no reporting module does.

Do we need an analyst to run it?

No — that's the design constraint we started from. Measures, risk models, and dashboards are maintained by Sansera; your team consumes answers and works the lists. If you have analysts, the API and export layer gives them everything raw.

Which quality programs are covered?

HEDIS, MIPS (including MVP pathways), Medicare Stars measure inputs, full UDS clinical tables, and Medicaid MCO contract measures — with state-specific variants handled in configuration.

Can it support our ACO / MSSP participation?

Yes — attribution tracking, benchmark and TCOC monitoring, shared-savings projection, and the quality-measure computation MSSP requires. The scenario modeler is built for exactly the "should we move up a risk track?" conversation.

Where does the risk-coding AI draw the line?

It surfaces evidence; it never codes. Every suggested condition routes to a provider for one-click confirmation with the supporting documentation attached — the compliant version of risk-capture, with an audit trail on every decision.

How fast to first value?

Feeds connect in weeks. Quality measures and risk scores compute as soon as history loads — most organizations see their first live gap lists and risk worklists inside the first month.

The Platform

The intelligence layer the rest of the platform runs on

SanseraPopHealth™ finds the patients; SanseraEngage™ reaches them; SanseraCCM360™ manages them and bills for it; SanseraScribe™ documents the work. One data platform underneath all of it — which is why insight becomes action here instead of becoming a slide.

Start with a working session, not a sales pitch.

Thirty minutes. Bring your worst quality measure and your riskiest contract — we'll show you what your data already knows about both.
