The monitoring program RPM can't touch — and therapists can bill.
Remote Therapeutic Monitoring is the most misunderstood program in the CMS care-management family, and one of the most valuable. It monitors therapy response — pain, function, adherence, symptoms — where patient-reported data counts, and where PTs, OTs, and SLPs can be the billing practitioner. If your organization does rehab, respiratory care, or behavioral health, RTM is revenue and clinical signal you're leaving on the table.
The Program
What Remote Therapeutic Monitoring actually requires
RTM (introduced 2022) pays for monitoring non-physiologic, therapeutic data in three anchor domains: musculoskeletal status, respiratory status, and cognitive behavioral therapy. Two structural differences from RPM change everything:
Self-reported data qualifies. A patient logging pain scores, exercise completion, or medication adherence into an app or a conversational SMS flow counts — no auto-transmitting device required.
Therapists can bill it. RTM was deliberately built so practitioners who can't bill E/M services — physical therapists, occupational therapists, speech-language pathologists — can run and bill remote monitoring.
The programs are complementary, not competing: a post-surgical patient can carry RPM with their PCP and RTM with their physical therapist in the same month — different practitioners, independent data, independent time. Sansera enforces the separation that makes it compliant.
How Sansera Runs It
Therapy-response monitoring that runs itself
01
Data capture patients don't abandon.
The AI agent collects pain scores, exercise completion, adherence, and validated outcome instruments through conversational SMS and app flows on the therapist's cadence — in the patient's language, at their literacy level. Every response day counts toward the 16-day rule, with self-report provenance recorded (the distinction an auditor checks).
02
Reads between the lines.
NLP on free-text responses catches what a form misses — a pain spike described in passing, confusion about the home exercise program, motivation slipping — and routes a coaching intervention or a therapist flag.
03
Therapist-owned episodes.
RTM episodes carry their own billing practitioner and their own time ledger, cleanly separated from any RPM or CCM running in parallel. Your PT's 98980 time never contaminates the PCP's 99457 time.
04
Outcomes on a schedule.
Validated pain and function instruments administered automatically at protocol intervals, trended, and charted into the therapist's documentation — clinical value and audit evidence from the same workflow.
05
Billing that walks the ladder.
Setup once, the correct device-supply code by monitored system, management codes only when the 20 minutes and the interactive contact are both evidenced — assembled and validated by the same Smart Billing Engine that runs every program on the platform.
FAQ
Remote Therapeutic Monitoring questions
Does the patient need a wearable or sensor?
Not necessarily. CMS treats qualifying software as a device for RTM purposes, and self-reported data collected through it counts. Sensor-based programs (motion trackers, connected inhalers) work too — the platform handles both provenances.
Can RTM and RPM be billed the same month?
Yes, by different practitioners with independent devices, data, and time — never by the same practitioner. Sansera validates practitioner separation before either claim releases.
Which conditions make the strongest RTM programs?
Post-surgical MSK recovery, chronic musculoskeletal pain with home exercise programs, COPD/asthma adherence, and behavioral-health engagement between sessions — anywhere therapy response between visits determines the outcome.
Is there really enough revenue here?
An active RTM patient typically generates ~$95–135/month across supply and management codes. For a rehab practice with a few hundred active episodes — or an FQHC with integrated behavioral health — it's a six-figure annual program that also tells your clinicians which patients are drifting off protocol.
SanseraCCM360™
One platform. Five programs. One right answer per patient.
CCM, PCM, and APCM are mutually exclusive — every patient, every month, there's one compliant program that pays best, and it changes as the patient changes. RPM and RTM stack on top with rules of their own. The Smart Billing Engine evaluates all of it monthly: eligibility, expected revenue, delivery requirements, and every compliance gate — deterministic rules, never AI guesswork, with an evidence bundle behind every claim.
