SanseraScribe™ · Ambient AI Documentation

The visit is the documentation. Everything else is automatic.

Your clinicians didn't train for a decade to spend their evenings typing. SanseraScribe™ listens to the visit — in the room, on video, in 25+ languages — and delivers a complete, structured note in your EHR before the next patient sits down. Codes suggested. Orders drafted. Instructions written for the patient. And because it runs on the Sansera platform, the documentation flows straight into billing — not into a pile.

See It

From conversation to signed note

Demo video coming soon — one continuous take: a short visit, the draft note appearing section by section, one edit, the suggested codes, a signature.

  • A real exam-room conversation — interruptions, small talk, a worried question
  • The structured note drafting itself: HPI, exam, assessment, plan
  • Every sentence traceable — tap any line to see the moment in the visit it came from
  • Suggested E/M level and diagnosis codes, waiting for the provider's confirmation
  • One edit, one signature, done
The Problem

Two hours a day. That's what documentation is taking.

The numbers are brutal and familiar: clinicians spend nearly two hours on the EHR for every hour of patient care, and finish charts at the kitchen table — the "pajama time" that turns into burnout, and burnout that turns into $500K–$1M physician replacements. Community clinics and FQHCs feel it worst: higher visit volumes, more complex patients, more non-English encounters — and no scribes budget.

The enterprise ambient tools proved the fix works. They also priced it for HCA. SanseraScribe brings ambient documentation to the organizations doing the hardest work on the thinnest margins — and connects it to billing, which the standalone scribes never do.

What It Does

Six things that happen while your clinician just... talks to the patient

The note writes itself

SanseraScribe captures the natural conversation — no dictating, no wake words, no changing how anyone talks — and produces a complete structured note: HPI, ROS as discussed, exam findings as verbalized, assessment, and plan. Specialty-aware templates (primary care, behavioral health, cardiology, pediatrics, OB, and more) and per-provider style learning mean the note reads like your clinician wrote it — because within a few weeks of edits, it effectively does. Every sentence is traceable. Tap any line in the draft and see the exact moment in the visit it came from. If the patient didn't say it and the clinician didn't say it, it isn't in the note — the Scribe documents what happened, never what it guesses.

Multilingual visits, documented in English

This is where safety-net care lives and where every enterprise scribe falls down. SanseraScribe handles the visit in the patient's language — Spanish, Vietnamese, Mandarin, Tagalog, Haitian Creole, 25+ in all — including mixed-language conversations and interpreter-mediated visits, and delivers the clinical note in English (or your documentation language). Your bilingual providers stop mentally translating while typing; your interpreter visits stop producing half-documented charts.

Codes suggested, billing connected

As the note completes, SanseraScribe suggests the E/M level with its supporting rationale and the ICD-10/CPT codes the documentation supports — validated against NCCI edits before a human ever sees them. The provider confirms with a tap; nothing is ever auto-coded. Then the platform does what no standalone scribe can: confirmed codes and documentation flow into the Smart Billing Engine, the same engine that runs your care-management billing. Document once, bill automatically — with an evidence trail from the spoken visit to the claim line.

The plan becomes orders

When the clinician says "let's get a CBC and I'll see you back in three months," the Scribe drafts the lab order, the follow-up scheduling task, and the referral if one was discussed — pended in the EHR for one-click signature. The plan stops being something someone has to remember to enter.

The patient gets instructions they can actually use

An after-visit summary in the patient's language, at their literacy level, covering what was discussed, what changed, and what to do next — with teach-back-friendly phrasing. The visit your patient understood shouldn't end with instructions they can't read.

The pre-visit brief

Before the clinician walks in: a one-screen AI summary of why the patient is here, what's changed since last visit, open care gaps, and medication changes — assembled from the same Sansera data platform. Two minutes of context that used to take ten minutes of chart-diving.

The Platform Difference

Standalone scribes end at the note. Sansera starts there.

A note that sits in the EHR is half the value. On the Sansera platform, the same visit powers the rest of your operation: confirmed codes flow to billing with documentation attached. Care-plan discussions update the CCM/APCM care plan in SanseraCCM360™. A closed care gap gets counted by SanseraPopHealth™ the same day. Follow-up scheduling routes to SanseraAccess™. One conversation, captured once, working everywhere — that's what "platform" means, and it's why the scribe pays for itself twice: once in clinician time, again in captured revenue.

How It's Built

Trust is the product

  • Consent, handled properly. Configurable consent workflows aligned to your state's recording rules, a visible recording indicator, and a one-tap pause for sensitive moments. Patients who decline get a normal visit and the clinician documents traditionally.
  • Audio doesn't linger. Recordings are processed and deleted on your retention policy — default measured in days, not months. No identifiable PHI is used to train our models.
  • Nothing enters the chart unsigned. Every note, code, order, and instruction is a draft until the clinician signs. Edit anything; the Scribe learns from it.
  • Works where you work. Phone in a pocket, web on a workstation, telehealth built in. Exam room, hospital round, home visit, video call.
  • Your EHR, not a new one. Notes land as structured documentation in Epic, Cerner, Athenahealth, eClinicalWorks, NextGen, and MEDITECH — discrete data where the EHR accepts it, not a PDF dump.
The Numbers

FAQ

SanseraScribe™ questions

How is this different from dictation?

Dictation makes the clinician do the work with their voice instead of their hands. Ambient means no work: the Scribe listens to the natural visit and structures the note itself. The clinician's job shrinks to review-and-sign.

What about hallucinations?

Every clinical statement in the draft is linked to the transcript moment or chart data that supports it — tap to verify. Anything the model can't support gets omitted, not invented; a visit detail the Scribe missed is a gap the clinician fills, which is recoverable. A fabricated finding is not — so the system is built to prefer omission, and we measure it continuously.

Do patients mind?

Overwhelmingly the opposite: they get a clinician who looks at them instead of a screen. Consent is explicit, the pause button is real, and declining is always fine.

Does it work for behavioral health visits?

Yes, with the sensitivity that specialty demands — therapy-aware note formats, strict minimum-necessary documentation, and 42 CFR Part 2 handling on the platform side.

Can our coders review before claims go out?

Yes — provider-confirmed codes can route through your coding team's queue before the Smart Billing Engine assembles claims. Your compliance posture, your workflow.

What does it cost compared to the big-name scribes?

Less — deliberately. We price for community health economics, and the billing connection means the Scribe funds itself from revenue the standalone tools leave on the table. Bring your visit volume to a working session and we'll show you the math.

The Platform

One conversation. Captured once. Working everywhere.

SanseraScribe™ turns the visit into the note, the codes, the orders, and the instructions — and feeds the platform that turns documentation into revenue and closed care gaps. That's the difference between buying a scribe and adding an AI workforce.

Start with a working session, not a sales pitch.

Thirty minutes. Bring one clinician who's tired of pajama-time charting — we'll run a live visit and hand them back the note.
